We provide DOT and CDL physical exams to maintain your driver certification as a commercial motor vehicle operator. Our exam meets the requirements of the Certified Federal Motor Carrier Safety Administration.

Examples of simple concerns include: suture/stitch removal, ear recheck, blood pressure recheck, refill of stable anti-depressant, etc.

These visits include moderately complex concern or 2 concerns- examples include abdominal pain, diabetes follow-up, new/worsening depression, COPD exacerbation, memory evaluation, follow-up of anxiety & thyroid disease, ADHD evaluation, etc. Healthy male & female annual physicals fall in this category.

These visits are for complex concerns or 3+ concerns – examples include migraine requiring in-clinic treatment, mental health counseling, follow-up of diabetes + high blood pressure + asthma + depression, mental health counseling, family meetings, annual physical + 1-2 other concerns, etc.

Prenatal Care through 28 weeks
This package includes all pregnancy-related visits for 7 months. Please note that Dr. Dall-Winther will hand your care off to a delivering physician @ 28 weeks.

4th Trimester Care
This package includes 6 visits – 1 in the 3rd trimester of pregnancy to discuss the final weeks, your birth plan, and the immediate postpartum period. From there, you and your baby will have 5 visits in your home (in-clinic visits available if preferred)at which we will spend as much time as it takes to address each of your physical, emotional & spiritual concerns.

Joint or Bursa Injection - Shoulder, Knee, Hip
Steroid injection can reduce pain and inflammation of the subacromial shoulder space from rotator cuff tendinosis or bursitis, knee joint arthritic pain or outer hip bursitis. Steroid use is a temporizing therapy, best utilized alongside a comprehensive physical therapy plan and activity modifications. It does not fix the underlying problem – rather the goal is to improve quality of life and function, reduce need for oral pain relievers, allow the body more time to heal while working through therapy measures or serve as a pain control bridge until surgical plans are completed for more significant troubles. It can reduce pain for several weeks to several months in many patients. Price includes brief pre-procedure consultation and procedure only. There is an $8.15 charge for the steroid medication itself.

Wart Removal
Most common warts go away without treatment, though it may take a year or two and new ones may develop nearby. Some people choose to have their warts treated by a doctor because home treatment isn’t working and the warts are bothersome, spreading or a cosmetic concern. The goals of treatment are to destroy the wart, stimulate an immune system response to fight the virus, or both. Treatment may take weeks or months. Even with treatment, warts tend to recur or spread. Cryotherapy is a freezing therapy done at a doctor’s office and involves applying liquid nitrogen to your wart. Freezing works by causing a blister to form under and around your wart. Then, the dead tissue sloughs off within a week or so. This method may also stimulate your immune system to fight viral warts. You’ll likely need repeat treatments, which are included in the price shown. Side effects of cryotherapy include pain, blistering and discolored skin in the treated area.

Skin Tag Removal
Often considered cosmetic and not reimbursable by insurance. These buggers pop up in all sorts of places – necklines, around the eyes, under the arms and breasts, inner thighs, etc. Base fee includes removal of up to 15 skin tags.

Partial Toenail Removal for Ingrown Nail
Recurrently painful, or inflamed and infected, nail edges from ingrown toenails? This procedure removes the edge that is stirring up trouble and aims to destroy the nail bed in this location. Doing so prevents regrowth and effectively narrows the total nail area, thereby reducing skin irritation and risk of infection.

Simple Laceration Repair (stitches)
No matter how careful we are, scrapes and cuts are bound to happen. In fact, for some people, small cuts can become so common that it’s difficult to know when an injury requires stitches and when it can just be bandaged at home.For very minor or shallow injuries, like paper cuts, simply clean the cut and apply a bandage, if necessary, to keep it clean.For more severe cuts, take the following action:
- Wash the cut with water and gentle soap to prevent infection.
- Stop the bleeding by applying gentle pressure to the injury.
- Examine the edges of the wound. If the edges are smooth and stay together during normal body movement, treatment is probably not needed.
Your wound may need stitches or other medical treatment if it meets any of the following criteria:
- The cut is deeper than a quarter of an inch.
- The cut was made by a dirty or rusty object and/or there is a risk of infection.
- Fat, muscle, bone, or other deep body structures are visible due to the wound.
- The cut is over a joint, especially if the edges open when you move the joint or if opening the wound shows muscle, bone, or joint structures.
- The injury is deep and on your hand or finger. •
- You have cosmetic concerns about the wound (i.e. it is on the face or area you don’t want to have a scar).
- After 15 minutes of pressure, the cut is still bleeding.
If you believe you may need medical attention, contact us ASAP. Wounds older than 6 hours will require more complex care. Prices shown are for simple cuts. Complex lacerations requiring multiple layers of stitches can be repaired for $350.
